REGISTRATION FORM To register for the Symposium and Workshop, please use the online registration form below. Website registration is strongly encouraged. Both credit card and cheque may be used for payment for online registrations. Please read the below terms and conditions before registering.

For those preferring not to register online, you may complete the printable registration form below and return it to the Symposium Organisers, either by fax or mail.


Each registrant must complete a separate form. Remember to send your completed registration form together with payment of your fees.
Please note that all persons, including speakers, intending to attend the Symposium and Workshop must register. Please read the below information carefully before you complete your form.

Day registration is available for any of the three days of the Symposium and Workshop. The Symposium is taking pace on Monday 12 and Tuesday 13 April 2010 and the Workshop on Wednesday 14 April 2010.

PAYMENT OF FEES
NOTE: Registrations will not be processed or confirmed until payment in full is received.
Payment may be made by cheque or credit card. Visa, MasterCard, American Express and Diners International are the only credit cards accepted at the Symposium and Workshop. Please note that all transactions by credit card will appear on your statement as payment to ‘The Meeting Planners'.
Cheques should be made payable to 'FMD 2010 / The Meeting Planners'. Your name and full address should be printed clearly on the back of the cheque. Cheque payments from overseas must be a bank cheque in Australian Dollars drawn on an Australian Bank. No other currency will be accepted.
Cheque payments must be received by 1 March 2010.
You may not pay your fees by direct transfer.
top of page ACKNOWLEDGEMENTS
Your registration and payment will be acknowledged via email or post with confirmation of your requirements according to your registration form. Your registration will not be processed or confirmed if payment does not accompany your form.
You will not receive any further publications before the Symposium and Workshop. Your letter of acknowledgment will include any further advice necessary prior to your arrival at the Symposium and Workshop.
top of page CANCELLATION AND REFUND POLICY
Cancellations received in writing at the Symposium Organisers' Office by 8 March 2010 will be accepted and all fees refunded less AUD100 administrative fee. Cancellations received after this date cannot be accepted and will not be refunded. However, transfer of your registration to another person is acceptable. The full name and details of the person that will replace you must be advised in writing to the Symposium Organisers' Office prior to the Symposium and Workshop. No refunds will be made for non-attendance at the Symposium and Workshop.
Registration Fees

Accommodation
- Deposit must be paid or credit card details provided at time of booking to guarantee accommodation booking
- Deposit is non-refundable at 8 March 2010
Social
- Payments are non-refundable as of 12 March 2009
ENTRANCE TO SYMPOSIUM
Each member of the Symposium and Workshop will receive a name badge on registration. The badge will be your official pass and must be worn to obtain entry to all sessions and to social functions.
INSURANCE
Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the Symposium and Workshop and book your travel you take out an insurance policy of your choice. The policy should include loss of fees/deposit through cancellation of your participation in the Symposium and Workshop, or through cancellation of the Symposium and Workshop, loss of international/domestic air fares through cancellation for any reason, loss of tour monies through cancellation for any reason including airline or related services strikes within Australia, failure to utilise tours or pre-booked arrangements due to airline delay, Force Majeure or any other reason, medical expenses (including sickness and accident cover), loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. The Symposium Organisers cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is to be purchased in your country of origin.
PRIVATE TOURING/HOSPITALITY ARRANGEMENTS
Should you have any specialised group requirements for private touring, transportation or hospitality services other than those organised for the official program, please contact the Symposium Organisers who would be pleased to assist with arrangements.
PRIVATE FUNCTIONS
It is requested that you advise the Symposium Organisers of any private/corporate functions that might be planned outside of the official program. The Symposium Organisers would be pleased to assist with arrangements for any of these events. top of page ACCOMMODATION

MLA's Welcome Reception at FMD 2010
Date: Monday 12 April 2010
Venue: Melbourne Convention and Exhibition Centre
An invitation is extended to all delegates to attend the Welcome Reception to be held at the Melbourne Convention and Exhibition Centre. Renew old friendships and make new acquaintances as we welcome you to Melbourne.
Tickets are included in the full delegate registration.
Additional tickets can be purchased for $60 per person through the online or printable registration form.
Symposium Dinner
Date: Tuesday 13 April 2010
Venue: to be advised
Join us for what promises to be one of the highlights of the Symposium. This will be a wonderful opportunity for colleagues and friends to get together in an informal setting and enjoy a wonderful dinner. Tickets are included in the full delegate registration.
Additional tickets can be purchased for $125 per person when you register through the online or printable registration form.
PRIVACY
Australia introduced the Privacy Amendment (Private Sector) Act 2000 in 2001. The Symposium Organisers comply with such legislation which is designed to protect the right of the individual to privacy of their information. Information collected in respect of proposed participation in any aspect of the Symposium and Workshop will be only used for the purposes of planning and conduct of the Symposium and Workshop and may also be provided to the organising body or to organisers of future events. It is also usual to produce a ‘Delegate List' of attendees at the Symposium and Workshop and to include the individual's details in such a list. Consent for publication of the individual's information may be withheld when completing the registration form for the Symposium and Workshop. Individuals are also entitled to access the information held by written application to the Symposium Organisers.
